In 1994, the residents of a small town Clear Lake became the victim of a strange and horrible disease. An influential local pastor called a strange disease the punishment for their sins. By the time the police and health officials arrived to find the cause of outbreak, thirteen people were missing, and most residents left the city. The government evacuated the remaining citizens and isolated town. Fifteen years later, the documentary filmmaker sought out the four surviving and persuaded them to return to Clear Lake to play their story, but after the return in the empty city, strange things start to happen again.
